Nursing care of a patient with grade Ⅳ myelosuppression complicated with septic shock

To summarize the nursing experience of a patient with grade Ⅳ myelosuppression complicated with septic shock. The key points of nursing included the prompting of cooperation with the first aid of septic shock,the close monitoring of infection indicators,the timely assisting for doctors to perform perianal abscess incision beside the bed,the accurate judge and thorough flushing of the abscess cavity under ultrasound guidance,the maintenance of effective drainage,giving patients improved total environmental protection quarantine measures,and the dynamical adjustment of blood glucose management strategies according to the changes of blood glucose. After systematic treatment and meticulous nursing,the patient was in stable condition after 9 days and transferred to the general ward for further treatment.

The relationship between health literacy,empowerment and quality of life in disabled elderly and their spouses based on the theory of “dyadic coping”

Objective To explore the relationship between health literacy (HL),empowerment and quality of life (QOL) in disabled elderly and their spouses based on the theory of “dyadic coping”. Methods From December 2020 to June 2021,498 disabled elderly and their spouses in Guangzhou,Zhuhai and Shenzhen of Guangdong province were surveyed by sociological data questionnaire,Katz Index ADL Scale,Health literacy Scale for chronic disease patients,Chinese Elders Health Empowerment Scale and Chinese Short Form-36 Health Survey. Spearman correlation analysis and structural equation model were used to analyze the data. Results The median scores of HL,empowerment and QOL of the disabled elderly were 86.00,30.00 and 391.39,respectively. The median scores of HL,empowerment and QOL of the spouses were 93.00,32.00 and 617.28,respectively. Correlation analysis showed that there were positive correlations between all variables(r=0.156~0.657,P<0.05),except correlation between spouses’ empowerment and QOL of the disabled elders(P=0.91). The structural equation model showed that the HL of the disabled elderly could affect the QOL directly or indirectly through empowerment (path coefficients were 0.30 and 0.23,P<0.001),and the model fitted well(GFI=0.811,AGFI=0.788,RMSEA=0.057). The HL of the disabled elders and their spouses had a positive interaction(path coefficient was 0.47,P<0.001). The QOL of the disabled elders was positively correlated with the QOL of their spouses(path coefficient was 0.18,P=0.004). Conclusion The interaction of HL and QOL between the disabled elders and their spouses suggests that the disabled elders and their spouses should not be considered separately under the pressure of disability,which verifies the rationality of the application of “dyadic coping” theory. The disabled elders and their spouses should be educated and empowered together to improve their QOL.

Nutritional risk prediction model for elderly hospitalized patients with mental disorders

Objective To construct a nutritional risk prediction model for elderly inpatients with mental disorders,and to provide a basis for clinical nutritional status assessment. Methods Using the method of cluster sampling,584 elderly patients with mental disorders who were hospitalized in Beijing Huilongguan Hospital from June 2020 to June 2021 were divided into a good nutritional status group(152 cases) and a malnutrition risk group(129 cases) and a malnutrition group(303 cases). The general condition questionnaire,frailty screening scale (FP),mini-nutrition assessment scale(MNA),simplified physical performance scale(SPPB),the Barthel Index(BI) of daily life and Family Care Index(APGAR) for ability evaluation were used for investigation. Logistic regression analysis and Graphpad Prism software were used to draw receiver operating characteristic curve to test the model fit and prediction effect. Results The incidence of malnutrition in elderly hospitalized patients with mental disorders was 51.88%,and advanced age(OR=1.049),frailty(OR=2.578),and BMI(OR=0.629) were independent risk factors for malnutrition risks(P<0.534). 0.05),in which advanced age was the greatest risk factor for malnutrition. Advanced age(OR=1.031),diagnosis(OR=2.150),frailty(OR=2.485),and physical dysfunction(OR=2.123) were independent risks of malnutrition(P<0.05),among which frailty is the greatest risk factor for malnutrition risk to be transformed into malnutrition. The area under the receiver operating characteristic curve(AUC) of the malnutrition risk is 0.732;the Youden index is 0.388;the optimal critical value is 0.473;the sensitivity of the prediction model is 0.736;the specificity is 0.651. Regarding the ROC curve of malnutrition,AUC is 0.808;Youden index is 0.531;the best critical value is 0.581;the sensitivity of the prediction model is 0.865;the specificity is 0.667. Those indicate that the 2 prediction models are accurate and reliable. Conclusion The risk prediction model of nutritional status of hospitalized elderly patients with mental disorders constructed in this study has high accuracy,and provides a basis for risk assessment and prevention of nutritional status of elderly patients with mental disorders in hospital.

Expert consensus of suicide care interventions for psychiatric inpatients with depression

Objective To form an expert consensus on suicide nursing interventions for inpatients with depression in psychiatric departments,standardize the contents of suicide nursing interventions for depression patients in psychiatric departments. Methods Evidence-based methods and literature analysis were used to extract recommendations and research conclusions of suicide nursing interventions for inpatients with depression,and the consensus draft was formed. After 2 rounds of expert letter consultation and 2 expert discussions,the draft was adjusted,modified and improved,and the final draft of the consensus was formed. Results The positive coefficients of the experts in 2 rounds of letter consultation were 100%;the authority degree of the experts was 0.924;the mean values of importance of each index were >3.5 points;the coefficient of variation was<0.25;the Kendall harmony coefficients of the experts were 0.182 and 0.260,respectively(P<0.01). Finally,the operational definition,suicide risk assessment,intervention form and time,theoretical basis of intervention,general principles of intervention,suicide impression intervention,suicidal behavior intervention,key points of intervention in special populations,evaluation of intervention effect and post-discharge evaluation of suicide care in hospitalized patients with depression were analyzed. A total of 11 components of the health education program were agreed upon. Conclusion The consensus provides the guidance for the suicide nursing interventions of psychiatric inpatients with depression,so that the suicide nursing intervention of psychiatric inpatients with depression is more standardized.

Construction of a core competency evaluation index system of organ transplant specialist nurses

Objective To construct the evaluation index system of core competence for organ transplant specialist nurses.Methods The first draft of evaluation index system was drawn up by referring to domestic and foreign literature,expert interviews and research group discussion. Totally 25 experts in China were consulted twice by Delphi method,and the weight of indexes at all levels was determined by analytic hierarchy process(AHP).Results The positive coefficients and authority coefficients for 2 rounds of consultation were 100% and 0.836. The coordination coefficient of expert opinions in the first,second and third level indexes were 0.404,0.449,and 0.308(P<0.001). The final evaluation index system of core competence of organ transplant specialist nurses included 7 first-level indexes(Theoretical Knowledge,Professional Skills,Critical Thinking Ability,Interpersonal Skills,Management Ability,Psychological Traits,and Specialty Development Ability),22 second-level indexes and 58 third-level indexes.Conclusion The evaluation index of core competence of organ transplant specialist nurses is logical and reliable,which can be used as reference for training organ transplant nurses.

Nursing care of 143 infants with iron deficiency anemia treated with iron agent before operation

This article summarized the nursing experience of 143 infants and children with iron deficiency anemia who received intravenous iron agent before operation. The key points of nursing included:proper preparation of liquid medicine,close observation of allergic reaction,rational selection of venous access,active prevention and treatment of phlebitis and leakage,and health education of iron treatment. All infants had no abnormal liver and kidney function during treatment,2 cases had increased heart rate,2 cases had phlebitis and 7 cases had pigmentation around the puncture site. These infants had normal heart rate,phlebitis was improved and pigmentation subsided after intervention. After intravenous iron treatment,the red blood cell count,hemoglobin,hematocrit,average red blood cell volume and average red blood cell hemoglobin of 143 infants increased significantly,and 112 received the surgery successfully after the iron treatment,while 24 continued the iron treatment,and 7 were lost to follow-up.
